
RESIDENTIAL HABILITATION
Description
Residential habilitation helps individuals develop daily living skills, including hygiene, homemaking, and social skills, based on a personalized plan. It is available for children and adults with specific limitations and cannot be combined with supported living coaching or in-home support services.
RESPITE CARE
Description
Respite care is a service that provides supportive care and supervision to a recipient when the primary caregiver is unable to perform these duties due to a planned brief absence, an emergency absence or when the caregiver is available, but temporarily physically unable to care for or supervise the recipient for a brief period.
SUPPORT EMPLOYMENT
Description
Supported employment services help individuals gain and maintain paid employment by providing training and assistance. This includes support in acquiring skills, finding jobs, and developing small businesses, with services tailored to the recipient’s preferences and needs.
SUPPORT LIVING
Description
Supported living coaching helps individuals live independently by providing training in daily skills, household management, and financial budgeting. This includes assistance with housing selection, safety assessments, and financial planning.
COMPANION CARE
Description
Companion services offer non-medical care, supervision, and social activities to help adults achieve goals in their support plan. These services focus on enhancing independence and community access, excluding hands-on medical care, and are not available under the DD Waiver – Tier Four.
SUPPORT EMPLOYMENT
Description
Supported employment services help individuals gain and maintain paid employment by providing training and assistance. This includes support in acquiring skills, finding jobs, and developing small businesses, with services tailored to the recipient’s preferences and needs.
BEHAVIOR ASSISTANT SERVICES
Description
Behavior assistant services offer one-on-one support for implementing and monitoring behavior analysis plans, focusing on individuals who need targeted interventions. These services are designed for short-term use to enhance independence and complement educational efforts for children.
IN-HOME SUPPORT SERVICES
Description
In-home support services provide up to 24 hours of assistance for individuals living independently, including personal care, daily living tasks, and companionship. Support workers may live with the recipient and work alongside supported living coaches, coordinating to avoid duplication.
PERSONAL CARE ASSISTANCE
Description
Personal care assistance helps recipients with daily activities like eating, bathing, dressing, and personal hygiene, as well as essential tasks such as meal prep and light housekeeping. This service is provided one-on-one and is not intended for supervision or as a substitute for meaningful day activities.
PERSONAL SUPPORT
Description
At Makwalla Healthcare Services, we have staff who are capable of providing clients with personal Support services. Whether you are recovering from illness or surgery, needing any short term relief, or living with a chronic disease such as stroke, cancer, or diabetes, our program for Personal Support can potentially delay or shorten your hospitalization. It can also be a less costly and more convenient alternative to admission in a care facility.
The individual stays at home and receive outstanding quality home care through our Personal Support program:
